The NHS is introducing a new way of storing and managing important medical information electronically, called Summary Care Records. Your Summary Care Record (SCR) is an electronic record of important information about your health. NHS Oldham has written to all our patients regarding Summary care records. You do not need to take any action, If we do not hear from you a Summary Care Record will be created automatically on the 10th of May

Please read and complete an opt out form and return it to the practice, If you do not a Summary Care Record will be created automatically.
